Life can change in an instant. Would your loved ones know what kind of healthcare you would want if you could no longer speak for yourself? Advance care planning is a process for planning ahead for future healthcare decisions, including choosing someone to speak for you when you can’t speak for yourself, communicating your choices to the person and documenting them in advance directive documents. This workshop will assist you in fully understanding the planning process as well as providing helpful information on completing advance directives.
What the workshop will cover:
What is Advance Care Planning and why is it important?
How to choose a healthcare agent (one who will make healthcare decisions when you are no longer able to do so)
The importance of reflecting on cultural religious or personal beliefs that may determine what type of care you would want
The importance of considering your desired quality of life and goals of treatment
Explaining the process of documenting wishes in the Advance Directives (Healthcare Power of Attorney and Living Will), including the differences between these documents
Briefly explaining what a DNR is
You can walk out with a Health Care Power of Attorney (that if witnessed properly) is valid in the State of Ohio.
Most importantly, emphasizing the importance of communicating your wishes to your loved one
